I agree and understand that registration and tuition payments are non-refundable. I understand that the registration fee is due at the time of registration to hold my dancer's spot in class or classes. I understand and give permission for my child to take classes at Equilibrium Dance Academy, LLC. I understand that there is a possibility of physical injury. I hereby waive, release, and forever discharge all rights and claims for damages which may arise now or in the future against any Equilibrium Dance Academy, LLC representative. I authorize Equilibrium Dance Academy, LLC to use their best judgement in any emergency. I hereby give permission for my child to be used in photographs and/or videos for promotional and marketing purposes. If you wish to cancel your enrollment, you may cancel by calling or emailing Equilibrium Dance Academy, LLC. I understand if I cancel my child's enrollment that fees are non-refundable but can be used as credit.
